【成份】

本品主要成分为盐酸地匹福林,含盐酸地匹福林应为标示量的90.0-115.0%。
化学名称为:2,2’-二甲基丙酸4-[1-羟基-2(甲胺基)乙基]-1,2-苯撑基盐酸盐。
分子式:C19H29NO5·HCl
分子量:387.90

【适应症】

治疗开角型青光眼和高眼压症。对闭角型青光眼虹膜切除后的残余性青光眼有效。对其他类型的继发性开角型青光眼和青光眼睫状体炎综合征也有效。

【用法用量】

一次1~2滴,一日1~2次,滴于结膜囊内,滴后用手指压迫内眦角泪囊部3~5分钟。

【不良反应】

地匹福林浓度仅为肾上腺素的1/10~1/20,因此不良反应的发生率要比肾上腺素低得多。溶液滴眼对血压和心率影响较小。但能引起散瞳(未经手术的闭角型青光眼禁用)和无晶体性黄斑病变。局部滴眼后有轻度烧灼和刺痛感,其他有滤泡性结膜炎、结膜血管收缩后反跳性充血、视物模糊、额痛及畏光和角结膜色素沉着等,停药后消失。全身不良反应一般不发生,偶有枕部疼痛、心律失常、心率增快、血压增高、脸色苍白、发抖和出汗等。

【禁忌】

未经手术的闭角型青光眼禁用。甲状腺功能亢进、高血压、冠状动脉供血不全、心律不齐、糖尿病等患者禁用。对本品过敏者禁用。

【注意事项】

1.无晶体性青光眼,高血压,心功能不全,甲状腺机能亢进者慎用。 2.由于妊娠期及哺乳期使用本品的安全性尚未确立,孕妇或哺乳期妇女应权衡利弊,在医生指导下方可使用。 3.对小儿的安全性尚未确立。

【特殊人群用药】

儿童注意事项:
对小儿的安全性尚未确立。

妊娠与哺乳期注意事项:
由于妊娠期及哺乳期使用本品的安全性尚未确立,孕妇或哺乳期妇女应权衡利弊,在医生指导下方可使用。

老人注意事项:
老年人视健康状况,慎用。

【药物相互作用】

与毛果芸香碱或b-肾上腺素受体阻滞剂联合应用有相加作用。

【药理作用】

地匹福林是肾上腺素的前药(Pro-drug),本身无生物活性,进入眼组织后在催化酶的作用下,迅速水解成肾上腺素而发挥生物效应,引起散瞳、降眼压。本品具有高度脂溶性,滴眼液滴眼后极易透过角膜屏障进入眼内。研究证明地匹福林的眼内通透性比肾上腺素强10~17倍。0.1%地匹福林的降眼压作用与1%肾上腺素相当,比2%肾上腺素略低,但散瞳作用与2%肾上腺素相当。降眼压作用与肾上腺素一样系通过减少房水分泌和改善房水流畅系数奏效。0.1%地匹福林溶液一滴滴眼后,30分钟开始降眼压,1~2小时获最大作用,维持约10~12小时。与毛果芸香碱或b-肾上腺素受体阻滞剂联合应用有相加作用。

This medicine promotes aqueous humor flow and lowers pressure in the eye by stimulating parasympathetic nerve. It is usually used to treat open-angle glaucoma or ocular hypertension.

This medicine lowers pressure in the eye by promoting aqueous humor outflow mainly by stimulating sympathetic receptor.
It is usually used to treat open-angle glaucoma or ocular hypertension.

Usage/Dosage

ophthalmic solution (Use-time dissolution type), white powder, limpid and colorless solution, (bottle) powder; brown vial, solution; deep yellow green, (cap) powder; vivid yellow green, solution; vivid yellow green-on-white double cap, limpid and colorless, ophthalmic solution, 5mL, (bottle) deep yellow green, (cap) vivid yellow green-on-white double cap

Cautions

•General dosage regimen: Dissolve the powder in the solution, and apply 1 drop of 0.04% preparation in your eye 1-2 times a day. If there is not enough effect, 0.1% preparation is used in the same manner. Following the instruction attached in the box, dissolve in the solution and shake well enough for use. Use the dissolved solution within 1 month. This medicine is 0.04% preparation. Strictly follow the instructions of your doctor/pharmacist.•Before using, wash your hands thoroughly with soap and running water. Next, remove yellow green cap of the dropper (Do not remove white cap). Then, pull down your lower eyelid so that the dropper tip does not touch eye or eyelashes. After dropping the solution into your eye, keep your eyes closed for 1-5 minutes pressing the inner corner of the eye lightly. Wipe off any excess liquid with a clean gauze or tissue. If you need to use other eye drops at the same time, wait more than 5 minutes.•Apply as soon as possible. If it is almost time for the next dose, skip the missed dose and continue your regular applying schedule. DO NOT use a double dose to make up for the missed dose.•If you used too much of this medicine (more than ordered), check with your doctor/pharmacist.•Do not stop using this medicine without the instructions of your doctor.

This medicine may cause sensitive to bright light or blurred vision. Do not operate dangerous machinery such as driving a car until these symptoms recover. If your eyes become more sensitive to bright light, wear sunglasses as necessary.

Contraindication

The most commonly reported adverse reactions include stinging eyes, eye pain, headache/headache dull, blurred vision, palpitation, pallor, sweating, tremor (tremble), and nausea/vomiting. If any of these symptoms occur, consult with your doctor or pharmacist.
The symptoms described below are rarely seen as initial symptoms of the adverse reactions indicated in brackets. If any of these symptoms occur, stop taking this medicine and see your doctor immediately.
